Visit any Medex Urgent Care center for a sports physical exam, often required for participation in athletic programs. To save time, fill out the “patient to complete” section on all necessary forms from your school, team, or sports program before your visit. If you can’t attend your child’s sports physical exam, complete the ‘History’ section of the form in advance. Remember, a sports physical assesses an athlete’s ability to safely engage in vigorous activity and is not a substitute for a general health maintenance exam.
